Unclear duplicate contacts are created

In the system, the phone number stands as a unique value
but once a week we clean up to 50 contacts with duplicates, how do they get where they come from?
Because of this, customers cannot create orders in the personal account, since an order is not created for a contact who has a double phone number....
1. Where does it come from
2. How can this be fixed?

all duplicates went through the REST API in requests for adding contacts
specify why you send them and from where
