I checked the creation of filters in the import according to the example of your file - I do not see any problems.
But I can say the following observation - you have multilingualism enabled, according to which the base language of the database is Russian.
The name column itself fills in the name of the Russian version of the filter. To fill in the Ukrainian version, I additionally recommend loading the nameua column
