There is a variable {|$managername|}, which immediately contains the manager's last name, but I need only the first name to be used for sending messages by email. There are no other variables where there is only the first name.

And in the variables, use the following construction

{|$custommanager_XXX|}, instead of XXX you need to substitute the key of the additional field
