Goods are not sent to the order from the personal account form

Made an edit:

The block was removed, but the goods that we select
for some reason they don't fit
Here is a sample of the order we received
/admin/customorder/order/144/edit/
added the following items

click on Submit

the goods you have chosen
not added to order

if we just use the form
/form/0/1/
then everything works fine

and here is the order with the goods that we received
/admin/customorder/order/145/edit/
